Quality Support, Inc. provides compensation packages that are competitive within our geographic markets. Competitive salaries are augmented by comprehensive benefits, which may include health care, retirement and savings plans.

General
Employment status with Quality Support can be full-time (FT), part-time (PT), or temporary. Continued employment is based on your ability to perform at your position and will be evaluated on a periodic basis by Quality Support management. Employment for assignments requiring a DOD clearance will also be contingent on your ability to obtain and maintain a DOD security clearance at the appropriate level.

Full-time employees are eligible for all of the company benefits. Part-time employees are eligible for Paid Time Off and Holidays (on a pro-rated basis), 401(k), Health Fitness Reimbursement and College 529 Plans. Temporary employees are not eligible for benefits except as required by state law.

This document is a general summary of Quality Support’s current benefits and is subject to change at any time at management's discretion. Specific policy and benefit details are available from the HR Department at Quality Support’s main office.


Payroll
The payroll schedule is bi-weekly, paid every other Friday. Pay weeks run from Saturday through Friday. Direct Deposit is offered and strongly encouraged.

BENEFITS

Paid Time Off
Quality Support recognizes the value of paid time off (PTO) for employees to be away from work for vacation, personal days or other reasons. Employees will earn PTO according to the following schedule, which changes on the anniversary date (example: a hire date of 1/1/2018 means the 5th anniversary date is 1/1/2023):

If earned, available PTO is not used by the end of the calendar year, employees may carry unused time forward to the next calendar year, up to the 400-hour maximum indicated above. If an employee has taken PTO more than their earned accrual (unaccrued leave/negative PTO balance) at the end of the year, he or she will be required to repay any such amounts Accrued, unused PTO is paid out at termination.

Holiday Pay
Quality Support provides ten paid holidays per calendar year to full-time employees. Quality Support follows the Federal Government holiday schedule.

Medical/Dental/Vision Health Insurance

Quality Support provides group health insurance to full-time employees through its partnership with Insperity PEO Services, L.P. Insperity provides health and dental coverage primarily through United Healthcare. Some work locations may have additional options available. VSP (Vision Service Plan) provides comprehensive eye care and defrays costs for glasses and contacts. If employees enroll in the health plan, coverage is effective on the date of hire. Employees must enroll online within 30 days of hire or wait for the next open enrollment period. Employee premiums are deducted from payroll on a pre-tax basis. Amounts vary depending on the coverage selected.

Employees may elect coverage for Employee Only, Employee plus Spouse, Employee plus Child, Employee plus Children or Employee plus Family. Domestic partners may be considered as spouses. Quality Support currently offers multiple health insurance plan options including PPO and High Deductible Health Plans (HDHP). Details about each plan including premiums and plan summaries are available from the HR Department.

Health Savings Account
Full-time employees enrolled in a High Deductible Healthcare Plan may participate in a Health Savings Account that acts as a tax sheltered, pre-tax savings plan for qualified medical expenses. Money is deducted from payroll pre-tax, grows tax free, and withdrawn tax free for qualified medical expenses. Investment options are available for account balances above the threshold. The IRS maximum annual contribution is $3,400 for individuals or $6,750 for families, plus a $1,000 catch-up max for employees over age 55. Unused amounts roll forward year-to-year.

Medical Flexible Spending Account
This account allows all full-time employees, regardless of participation in the group health insurance plan, to elect to contribute a portion of their earnings on a pre-tax basis through payroll deductions, for reimbursement of eligible health care expenses (those that are not covered by medical insurance, such as co-pays, deductibles, eyeglasses, etc.). The maximum annual contribution is $2,600. All contributions must be used within the calendar year or they will be forfeited

Group Life and AD&D Insurance
Quality Support provides group life insurance and accidental death and dismemberment (AD&D) coverage to all full-time employees up to $50,000 per year or 1x salary, whichever is lower. The plan is offered through Cigna.

Voluntary Life and AD&D Insurance
Full-time employees may purchase supplemental Voluntary Life and AD&D insurance in an amount up to 6x annual earnings with a $2.5 million maximum (subject to evidence of insurability over $500,000). Spouse/domestic partner coverage is available at half the employee rate up to $200,000 (subject to insurability over $30,000). Dependent children may be covered at $5,000 or $10,000 flat dollar amounts. Payroll deductions are taken post-tax. The plan is offered through Cigna.

Short Term & Long-Term Disability
Quality Support provides short-term and long-term disability to all full-time employees. The benefit amount is the lesser of 60% of your covered earning rounded to the nearest dollar or maximum benefit amount ($2,308 per week or $10,000 per month). Short-term disability begins on the fifteenth day of a disability with a maximum period of 24 continuous weeks. Long-term disability would then begin after 180 continuous days of disability and continue to normal social security retirement age, if required.

401(k) Plan
Quality Support provides a 401(k) Retirement Plan through Insperity. All full-time and part-time employees who are at least 21 years of age and have been employed for 12 months and have worked at least 1000 hours are eligible to enroll in the 401(k) plan. Quality Support matches the employee’s contribution dollar for dollar up to a maximum amount of 4% of compensation per pay period. The 401(k) plan offers over 20 investment options and an option for a self-directed brokerage account. Participants may elect to make Roth 401(k) (post-tax) and/or Regular 401(k) (pre-tax) contributions to the plan.

Health & Fitness
Quality Support will provide an allowance up to $240.00 per year for an employee’s individual health/fitness center/club membership. This benefit is available to all full-time and part-time employees. Health/fitness monetary assistance is not transferable and cannot be accumulated. This is a taxable benefit.

Employee Assistance Program
To help employees and their families balance work and personal issues, Quality Support offers a flexible and comprehensive employee assistance program – at no cost to the employee. This benefit provides confidential support and advice, customized resources and referrals and practical information on hundreds of work and life topics. Employee need not be enrolled in other coverage to use this benefit.

Educational Assistance
To encourage eligible employees to voluntarily acquire and maintain technical and professional competency and awareness of advances in the field of employee’s specialization and to improve skills, Quality Support offers employees educational assistance. Through Insperity, up to $1,500 per calendar year for approved undergraduate or graduate college courses and $500 per year for approved continuing educational expenses is available. This may also be used towards certification preparation. Additionally, Quality Support matches this program for a total of $3,000 per year in tuition allowance and $1,000 per year for continuing education.

Adoption Assistance
Adoption assistance is provided for active, full-time employees who have completed 6 months of service. A maximum of $1,500.00 will be reimbursed provided all guidelines are met. This benefit is not contingent upon participation in the medical plan.

529 College Savings Plan

Full-time and Part-time employees are eligible to enroll in the College America Virginia 529 College Savings Plan. Employees enroll directly with American Funds to contribute after-tax funds (which are invested and grow tax-free) for reimbursement of future college tuition payments. Enrollment, plan administration and premium payments are coordinated directly with American Funds.

Pet Insurance

Since pets are considered more of a family member today than ever, we have options for employees to purchase pet insurance through the Insperity MarketPlace SM at a discounted rate.

Employee Referral Bonus Plan
Employees that provide qualified referrals that result in a company hire will receive a $1,000 referral bonus after the referred employee has complete 6 months of employment. The referred employee is required to document the employee that provided the referral at the time of hire/ during the onboarding process for the referral qualification to be met.

Commuter Benefits
Quality Support offers commuter benefits through Insperity and administered through WageWorks®. The program allows participants to save up to 40% on transportation costs by paying for mass transit and/or parking expenses with pretax dollars.

Participation in the Program is open to any full-time or part-time employee, regardless of the number of hours worked per week. The Commuter Benefits Program has no initial or annual open enrollment period -- employees may elect to enroll in commuter benefits at any time and may discontinue participation at any time as well.
